Application for Non-CITES Wildlife Products - Import

 

Requirements

1. Duly accomplished application form
2. Export Permit/Certification of Origin from exporting country
3. For live specimens, Phytosanitary Certificate (for plants) or Veterinary Quarantine Certificate (for animals) issued by the authorized government agency of the country of origin
4. Payment Fee in accordance with DAO 2004-55 Section 9
